NAD+, NMN, and the longevity supplement question nobody has fully answered yet

NAD+, NMN, and the longevity supplement question nobody has fully answered yet

Key takeaways

  • NAD+ (nicotinamide adenine dinucleotide) is a critical coenzyme in cellular energy production, DNA repair, and sirtuin activation. It declines measurably with age.
  • NMN and NR are oral precursors confirmed in multiple randomized controlled trials to raise blood NAD+ levels by 40 to 100 percent in adults over 40. What happens downstream of that raise is less settled.
  • The animal data on NAD+ precursors are extraordinary. The human clinical outcomes data are promising but not yet definitive.
  • Running a protocol without baseline bloodwork is guessing. Track whether it is achieving measurable results before committing to a long-term spend.

The longevity supplement with the best mechanism. Probably

NAD+ sits at the center of cellular energy metabolism. It shuttles electrons through the mitochondrial chain, activates sirtuins (the longevity proteins), supports PARP in DNA repair, and modulates circadian clock gene expression. By age 60, human NAD+ levels are roughly half what they were at age 20. This decline is real, measurable, and mechanistically connected to the cellular hallmarks of aging.

NMN and NR are oral NAD+ precursors that have been shown in published randomized controlled trials to raise blood NAD+ levels in healthy adults over 40. The supplements are real. The pathway is real. What is still being worked out: whether raising blood NAD+ levels produces the specific clinical benefits suggested by animal studies, at the doses and timescales available to a human taking a daily capsule.

The people selling $150-per-month subscription stacks are getting ahead of what the clinical trials have confirmed. That gap is worth knowing before spending the money.

What the human trials have actually shown

Blood NAD+ elevation. Confirmed in multiple RCTs for both NMN and NR. A 2022 study in Cell Reports Medicine showed NMN increased blood NAD+ by approximately 38 percent in overweight middle-aged adults over 10 weeks. The most consistent finding across the literature.

Muscle function and insulin sensitivity. The same 2022 trial showed improved insulin sensitivity in skeletal muscle in post-menopausal women with prediabetes or obesity. A separate 2023 trial showed improved exercise endurance. Specific, measurable outcomes in specific tissues.

Cognitive function and subjective energy. Mixed results across trials. Some show improvement in subjective fatigue scores. Others show no significant difference versus placebo. Longevity marker data in humans remain sparse.

Safety. Both NMN and NR appear safe at the doses studied, with no serious adverse events reported in trials lasting up to 12 months.

NMN versus NR and how to choose

Both raise blood NAD+. NR has been studied longer and has more published human trials. NMN is one biosynthetic step closer to NAD+. The head-to-head clinical data do not clearly favor one over the other.

For NR, Tru Niagen 300 mg is the most extensively studied brand, with published human trial data for its specific formulation. For NMN, Jarrow NMN 125 mg is a reliable, lower-cost entry point. Start with one, run a clean 12-week trial, assess, then adjust.

PQQ (pyrroloquinoline quinone) acts via a complementary mechanism, supporting mitochondrial biogenesis rather than directly affecting NAD+ levels. The two address different nodes of the same system and can be stacked without redundancy. Doctor’s Best PQQ 20 mg is the dose used in published trials. Life Extension NAD+ Cell Regenerator is a comparable NR alternative for those who want a second option.

The Livium recipe

Tool. Run a comprehensive baseline panel before starting, covering methylation markers, fasting insulin, and HbA1c. Pair with a wearable to track HRV and readiness as proxy outcome markers across the 12-week trial. Rerun the panel at 12 weeks to see what moved.

Behavior. 300 mg NR or 250 mg NMN taken in the morning. NAD+ has circadian activity, and morning dosing aligns with the metabolic window. Run for 12 weeks without changing any other major variable. Changing sleep, diet, and exercise simultaneously makes it impossible to know which one moved the marker.

Threshold. At 12 weeks: HRV trend up or stable, subjective energy measurably improved, relevant bloodwork markers moved. If none of those are true, the cost-benefit calculation does not support continuing. Try the other precursor before abandoning the category.

The honest evidence table

Claim Evidence level Livium take
NMN/NR raises blood NAD+ Strong (multiple RCTs) Confirmed
Improves insulin sensitivity in muscle Moderate (limited RCTs, specific populations) Plausible; worth tracking
Improves subjective energy Weak (mixed results, small trials) Possible, but verify with data
Extends human lifespan Strong in mice; unverified in humans Mechanistically plausible; not yet proven
Safe at standard doses Strong (12-month trials, no serious AEs) Confirmed for near-term use

Plan of action

  • Before spending money here, confirm that ferritin, testosterone, B12, and thyroid are all optimized. Fix those first. Cheaper, better evidence, more predictable results.
  • Start with NR if trying this category for the first time. Take it in the morning. Track HRV on a wearable throughout the 12-week experiment.
  • If HRV does not trend upward by week 12, the intervention is not producing the cellular energy effect at that dose. Try the other precursor, or reconsider whether foundational markers need to be addressed first.

FAQs

Is NMN FDA-approved? +

No. In late 2022, the FDA issued a decision that NMN does not qualify as a dietary supplement because it was under investigation as a drug. This decision is being contested. NR remains available without this regulatory complication.

Can NMN and NR be stacked? +

Theoretically, no harm, but no evidence of additive benefit versus either alone at studied doses. Run one as a clean experiment before stacking.

How long does it take for NMN or NR to raise NAD+ levels? +

Most studies show measurable increases in blood NAD+ levels within a few weeks of consistent supplementation. However, changes in energy, exercise performance, or metabolic markers tend to take longer and are less predictable. A 12-week trial is generally enough time to determine whether the supplement is producing meaningful changes in wearable metrics, bloodwork, or subjective energy levels.
