The PHQ-9 cannot distinguish burnout from depression. The questionnaire measures symptom overlap. The clinician who does not ask about work context, cortisol pattern, and the specific domains where function has collapsed will miss the distinction that determines whether the treatment should be an antidepressant, an HPA-axis recovery protocol, or a work structure change.

The neurobiological distinction

Burnout represents a late-stage HPA axis response to chronic, unrelenting demand. The initial phase of chronic overwork produces elevated cortisol as the HPA axis attempts to maintain performance under load. Over months to years, the HPA axis fails to maintain that elevated output and begins to flatline. The morning cortisol awakening response blunts. The diurnal curve flattens. The person who used to be energized by their work now wakes unrested and cannot generate the cortisol-driven alertness that previously carried them through demanding days.

Depression, by contrast, more commonly produces elevated cortisol, particularly elevated evening and nocturnal cortisol, reflecting ongoing HPA axis activation rather than HPA axis depletion. The clinical differentiators

Context specificity. Burnout is work-specific. The person with burnout who goes on a two-week vacation often experiences substantial recovery of mood, energy, and pleasure. The person with depression typically does not. If a forced break from work produces meaningful symptomatic improvement, burnout is the more likely diagnosis.

Anhedonia pattern. Depression produces global anhedonia: the inability to experience pleasure in anything. Burnout produces domain-specific anhedonia: work-related activities feel joyless while non-work activities can still produce positive affect.
